These canary laboratory report sheets, measuring 8-1/2″ x 11″, are specifically designed for the collection and organization of patient data. Each sheet is equipped with horizontal adhesive strips, accompanied by liners on the top and bottom, allowing easy attachment of lab reports for the collection and organization of patient information once the liner is removed.

Our BF-C-5 laboratory mount sheets are tailored to hold up to five reports. They feature a 5-hole punch at the top, following the standard hospital punch pattern, enabling convenient attachment of the forms inside the patient’s medical chart. These mount sheets find their ideal application in medical facilities such as hospitals, clinics, urgent care centers, and rehabilitation centers.
